If someone has been raised and confirmed in the Lutheran church and wants to become a Catholic, does he have to be confirmed again?

No, he doesn't have to be confirmed "again"; rather, he needs to actually receive confirmation. He was not actually confirmed in the Lutheran church because Lutheran ministers do not have valid holy orders. When he is confirmed in the Catholic Church, he will be validly confirmed -- not "re-confirmed."
